Testosterone Replacement Therapy (TRT) Optimization
Testosterone Replacement Therapy (TRT) Optimization
TRT stands for testosterone replacement therapy, also known as androgen replacement therapy. It is primarily used to treat low testosterone levels, which can decrease with age or due to medical conditions.
TRT is also gaining popularity for non-medical uses, including:
- • Enhancing sexual performance
- • Increasing energy levels
- • Building muscle mass for bodybuilding
While some research supports the benefits of TRT in achieving these goals, there are important considerations. Let’s explore how testosterone levels change with age and what you can realistically expect from TRT.

As you age, your body naturally produces less testosterone. According to American Family Physician, the average male’s testosterone production decreases by about 1 to 2 percent each year. This process typically starts in the late 20s or early 30s:
- The testicles produce less testosterone.
- Reduced testicular testosterone causes the hypothalamus to produce less gonadotropin-releasing hormone (GnRH).
- Lowered GnRH leads to less luteinizing hormone (LH) production by the pituitary gland.
- Reduced LH results in overall lower testosterone production.
This gradual decline usually doesn’t cause noticeable symptoms, but a significant drop in testosterone levels can lead to:
- Low sex drive
- Fewer spontaneous erections
- Erectile dysfunction
- Reduced sperm count or volume
- Difficulty sleeping
- Unusual loss of muscle and bone density
- Unexplained weight gain
The only way to determine if you have low testosterone is by consulting a healthcare provider for a testosterone level test. This simple blood test is necessary before starting TRT and may need to be repeated to account for factors like:
- Diet
- Fitness level
- Time of day the test is conducted
- Certain medications, such as anticonvulsants and steroids
